ABSTRACT

El síndrome de Cornelia de Lange (SCdL) es un trastorno genético poco frecuente y se atribuye principalmente a mutaciones en los genes NIPBL, SMC3 y SMC1A. Sus principales características clínicas son múltiples anomalías congénitas, dimorfismo facial, hirsutismo, hipertricosis, retraso psicomotor, discapacidad intelectual, restricción del crecimiento prenatal y postnatal, anomalías de manos y pies, así como malformaciones congénitas que afectan a distintos órganos. En pacientes con SCdL es necesario hacer hincapié en la higiene oral debido a la discapacidad intelectual que puede presentarse y asegurarse de que se realiza una adecuada valoración y saneamiento dental de forma periódica con el fin de prevenir enfermedades bucodentales. El objetivo de este reporte de caso es describir el manejo odontológico de un paciente de 10 años con SCdL y revisar las características clínicas y hallazgos radiológicos presentes en la cavidad oral (AU)


Cornelia de Lange syndrome (CdLS) is a rare genetic disorder and is principally attributed to mutations in the NIPBL, SMC3 and SMC1A genes. The main clinical characteristics are multiple congenital anomalies, facial dimorphism, hirsutism, hypertrichosis, psychomotor retardation, intellectual disability, prenatal and postnatal growth restriction, hand and foot anomalies, as well as congenital malformations affecting different organs. In patients with CDLS, it is necessary to focus on oral hygiene due to the intellectual disability that may be present and to ensure that adequate dental valuation and hygiene is routinely performed in order to prevent oral diseases. The aim of this case report is to describe the dental management of a 10-year-old patient with CDLS and review the clinical characteristics and radiological findings that are present in the oral cavity (AU)

ABSTRACT

Introduction: Aspergillus fumigatus is the most prevalent fungal pathogen reported to cause diseases such as aspergilloma or aspergillosis in humans. Aspergillomas are commonly seen in a poorly drained and avascular cavitary space. Paranasal sinuses are most commonly involved, especially maxillary sinus. In the past two decades, the incidence of aspergillosis has increased substantially. Aims: To evaluate cases reported as aspergilloma of the maxillary sinus and to determine the percentage of cases involving aspergilloma of the maxillary sinus in healthy individuals. Materials and Methods: After the final full-text review,16 articles were included in this systematic review. Data extracted from these full-text articles was reviewed. Results: 83 % of cases had a history of dental procedures, with 42 % of those being due to infection from previous extraction sockets and 41 % due to root canal therapy (RCT). About 43% of the patients were immunocompromised, while 56% were healthy without any predisposing conditions. Conclusion: Aspergillus fungal infections of the paranasal sinuses are common and can occur in apparently healthy as well as immunocompromised individuals. Aspergilloma is the most common fungal infection involving the maxillary sinus with iatrogenic-dentogenic factors being predominant for initiation and progression of the infection. About 43% of the patients in this review were immunocompromised patients whereas 56% of the patients were healthy without any known predisposing condi- tions. The progression and prognosis of this disease depends on the location and immunologic status of the patient. So, it is very important for dentists to be cautious while performing any dental procedures so as not to initiate any iatrogenic infections.

ABSTRACT

O manejo odontológico adaptado ao paciente que é diagnosticado com câncer é fundamental para a melhoria e manutenção da qualidade de vida dos mesmos. Esses indivíduos devem ser submetidos a uma avaliação odontológica prévia ao tratamento anticancerígeno, a fim de estabelecer as etapas necessárias às intervenções e o que pode ser realizado antes, durante e pós-tratamento, sendo essas fases executadas na clínica diária ou até mesmo no ambiente hospitalar, exercendo um trabalho multidisciplinar. A presente revisão de literatura tem como objetivo direcionar a atenção dos cirurgiões dentistas para o planejamento e atendimento personalizado dos pacientes oncológicos, de acordo com suas necessidades e particularidades.


Dental management adapted to patients diagnosed with cancer is essential for improving and maintaining their quality of life. These individuals must undergo a dental evaluation prior to anticancer treatment, in order to establish the necessary steps for interventions and what can be done before, during and after treatment, these phases being performed in the daily clinic or even in the hospital environment, exercising multidisciplinary work. This literature review aims to direct the attention of dentists to the planning and personalized care of cancer patients, according to their needs and particularities.


El manejo odontológico adaptado a los pacientes diagnosticados de cáncer es esencial para mejorar y mantener su calidad de vida. Estas personas deben someterse a una evaluación odontológica previa al tratamiento anticanceroso, con el fin de establecer los pasos necesarios para las intervenciones y lo que se puede hacer antes, durante y después del tratamiento, siendo estas fases realizadas en la clínica diaria o incluso en el ámbito hospitalario, ejerciendo un trabajo multidisciplinar. Esta revisión bibliográfica pretende dirigir la atención de los odontólogos a la planificación y atención personalizada de los pacientes con cáncer, de acuerdo con sus necesidades y particularidades.

@#Oxidative stress is closely associated with the development of oral diseases such as caries, periodontitis and endodontitis. The accompanying oxidative stress during inflammation could aggravate tissue damage. However, numerous studies have shown that some dental materials, such as composite resins, bleach, drugs for root canal irrigation and dental implants, can give rise to abundant free radicals, which have adverse effects on peripheral tissues. Therefore, it is essential to supplement with extra antioxidants against free radicals. Plant-derived natural antioxidants have attracted great attention in biomedicine because of their excellent biocompatibility and easy access. This paper focused on the redox imbalance in the oral cavity and the application of natural antioxidants to oral therapy and their modification of dental materials. Current research shows that by constructing polyphenol-based metal organic nanoenzymes or adding vitamins and polyphenols to bionic hydrogels, the safety and utilization rate of antioxidants can be significantly improved. However, these polymer delivery systems have problems such as poor degradability, hepatotoxicity and nephrotoxicity, and the research is still in its infancy. In terms of material modification, it is crucial to choose the type and ratio of natural antioxidants and raw materials, as well as appropriate modification methods. A strong chemical bond between the antioxidant and the raw material may lead to the failure of antioxidant release from the modified composite, lowering the antioxidant activity. At the same time, the selection of polyphenols rich in pyrogallol functional groups can retain more free phenolic hydroxyl groups after chemical modification, which is conducive to greater antioxidant activity by the implant materials. Although research on natural antioxidants in oral therapy has made progress, there is a lack of data supporting clinical trials and long-term application effects, and further research is still needed.

Objective @# To investigate the efficacy of a holistic approach for postoperative pain management in children receiving dental treatment under general anesthesia in day-surgery operating room. @*Methods@#A total of 120 children, aged 3-7 years, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, with ≥ 10 treated teeth, receiving comprehensive dental treatment under general anesthesia from January 2020 to August 2020 were enrolled in this trial and randomly allocated into the holistic approach group (group H, n=60) and including preemptive analgesia, instructions to parents for pain management and web-based assessment system (assessment pain by scanning the quick response code 4, 6, and 24 hours postoperatively) and the control group (group C, n=60) only scanning the quick response code. Pain, face, legs, activity, cry and consolability (FLACC) scale was used to assess the level of pain 2 h postoperatively and the parents postoperative pain measure (PPPM) was used to assess the level of pain 4, 6, and 24 h postoperatively in two groups.@* Results@# The FLACC scores of group H 2 h postoperatively were significantly lower than group C (P <0.05). The incidences of significant pain (PPPM scores ≥ 6) 4, 6 and 24 h postoperatively in group H were lower than group C (P <0.05). Altogether, 91.7% of parents in group H and 71.6% in group C assessed the level of pain of children over time. The compliance rate of parents in group H was significantly higher than group C (P <0.05).@*Conclusion @# The holistic approach had a positive effect on reducing postoperative pain for children receiving dental treatment under general anesthesia in the day-surgery operating room.

ABSTRACT

Abstract The ingestion or aspiration of dental material or instruments is one of the most feared complications in clinical dental practice, it can occur in both children and adults. The mouth is a moist, dark, small area; where the involuntary movements of the patient during the procedures and the small dimensions of the materials and instruments turn dental care into a challenge. A swallowed or aspirated foreign body during dental treatment can result in serious complications and even death, these depend largely on the shape, size and anatomical pathway through which it passes. This article describes an uncommon complication of which there is only one report in the literature worldwide, the ingestion of the tip of a triple syringe during a routine dental procedure that was recovered by esophagogastroduodenoscopy. It also highlights the importance of knowing the protocols for handling aspirated or swallowed foreign bodies where a fast and adequate approach during the first minutes can make the difference between life and death.

Introducción: Las urgencias médicas pueden ocurrir durante la práctica estomatológica. Objetivo: Abordar elementos actuales sobre la epidemiología, prevención, cuadro clínico y tratamiento de las urgencias médicas más frecuentes durante la atención estomatológica. Material y Métodos: Se realizó una revisión bibliográfica en septiembre de 2021. Se evaluaron revistas de impacto de Web of Science (19 revistas). Se consultaron bases de datos de MEDLINE, Google Académico, PubMed y SciELO con los descriptores: "medical emergency", "odontology treatment", "epidemiology". Se incluyeron artículos en idioma español e inglés, mayoritariamente de los últimos 5 años. Los que mejor cumplieron los criterios de revisión fueron 31 artículos y fueron incluidos como referencias en este artículo. Desarrollo: Las urgencias más frecuentes son las lipotimias y los síncopes vaso vágales. El diagnóstico basado en los antecedentes patológicos y hallazgos clínicos permite tomar conductas adecuadas. La terapéutica actual aboga por la protocolización de la atención y el uso justificado, sustentado por el conocimiento, de un botiquín de medicamentos de urgencia. Conclusiones: Las urgencias médicas pueden ocurrir durante la práctica estomatológica y poner en riesgo la vida de los pacientes. Su incidencia es variable, el riesgo para la vida no es frecuente y la sobrevida es elevada. Confeccionar una adecuada historia clínica y preparar al personal evitará complicaciones posteriores(AU)


Introduction: Medical emergencies can occur during dental practice. Objective: To address current issues in the epidemiology, prevention, clinical picture and treatment of the most frequent medical emergencies during dental treatment. Material and Methods: A literature review was conducted in September 2021. Web of science impact journals (19 journals) were evaluated. Medline, PubMed, Google Scholar, and SciELO databases were consulted using the following descriptors: "medical emergency", "odontology treatment", and "epidemiology". Articles in Spanish and English were included, preferably those published within the last 5 years. A total of 31 articles were obtained and included as references in this article. Development: Lipothymia and vasovagal syncope are the most frequent emergencies. The diagnosis based on pathological antecedents and clinical findings allows us to take appropriate behaviors. The current therapy advocates for the protocolization of care and justified use, supported by knowledge about a first-aid kit. Conclusions: Medical emergencies can occur during dental practice, putting patients´ lives at risk. The incidence is changeable and the survival rate is high. The development of an adequate clinical history and training of the staff will prevent further complications(AU)

RESUMEN Como consecuencia del alargamiento de la vida, aumenta también el número de adultos con discapacidades y enfermedades crónicas degenerativas, que pueden causarlas. El objetivo de esta revisión es conducir un análisis de las evidencias disponibles sobre la importancia de la atención estomatológica al paciente geriátrico con dismovilidad. Se tuvo en cuenta las características del adulto mayor dependiente, la atención domiciliaria como actividad asistencial y la atención odontológica domiciliaria que arrojó como conclusiones que para garantizar una buena salud bucodental de los adultos mayores con dismovilidad, se requiere de una correcta valoración geriátrica y un trabajo en equipo donde los estomatólogos juegan un papel importante ya que la atención estomatológica demanda de una asistencia odontológica domiciliaria provista de los elementos necesarios para su funcionamiento.


ABSTRACT As a result of the lengthening of life, the number of adults with disabilities and chronic degenerative diseases, which can cause them, also increases. The objective of this review is to conduct an analysis of available evidence on the importance of stomatological care to the geriatric patient with dysmobility. The characteristics of the dependent elderly were taken into account, the home care as a health care activity and the home dental care that was given as conclusions to ensure good oral health in the elderly with dismobility, proper geriatric assessment and teamwork are required where stomatologists play an important role as stomatological care requires home dental care provided with the necessary elements for its operation.

ABSTRACT

@#Autism Spectrum Disorder (ASD) is a neurodevelopmental disorder characterized by persistent deficits in social communication and the presence of restricted interests and repetitive behaviours. Comorbidities following ASD, such as seizure, intellectual disability, and sensory impairment worsen patients’ ability to care for themselves. We present the case of a 22-year-old man with autism, intellectual disability and visual impairment who had recurrent pain in his upper and lower left posterior teeth that had cavities. On the first visit, the patient was observed and had panoramic x-ray. Clinical examination could not be done properly due to lack of patient cooperation. Restoration, pulp capping, tooth extraction, and odontectomy were planned under general anaesthesia.

Introdução: A doença cárie possui uma etiologia multifatorial que pode atingir toda a estrutura dentária. Em cavidades profundas, o tratamento expectante pode ser realizado com a finalidade de evitar danos à polpa. Já em lesões iniciais, a técnica ART é uma alternativa aceitável para o caso. Objetivo: Discutir sobre as indicações, contraindicações, técnicas e relevâncias clínicas do tratamento expectante (convencional e modificado) e da técnica restauradora atraumática (ART) no tratamento das lesões cariosas. Métodos: Foi realizada uma revisão da literatura de artigos científicos publicados nas bases de dados PubMED/Medline e Scielo, entre 2009 e 2020, e de livros considerados relevantes para este estudo. Resultados: No tratamento expectante, a remoção da dentina cariada é realizada em duas etapas: remoção da lesão cariosa de forma superficial, na primeira consulta, e remoção final após diferentes intervalos de tempo. No entanto, alguns autores defendem que não é necessária a realização da segunda etapa, basta apenas rebaixar o cimento de ionômero de vidro restaurador e, em seguida, realizar a restauração definitiva, técnica nomeada como tratamento expectante modificado. Com relação à técnica ART, o manejo da lesão é feito com a mínima intervenção possível, utilizando apenas instrumentos manuais. É confiável e bem aceita, principalmente na clínica de Odontopediatria. Ambas culminam em resultados terapêuticos favoráveis. Conclusão: Inicialmente, o dentista deve sempre realizar um diagnóstico minucioso das lesões de cárie, visando a máxima prevenção da estrutura dental. Devemos optar pelo tratamento restaurador convencional somente quando outros métodos não forem mais suficientes para interromper a progressão da lesão.


Introduction: Caries disease has a multifactorial etiology and can affect the entire tooth structure. In deep cavities, expectant treatment can avoid damage to the pulp. In early lesions, the ART technique is an acceptable alternative. Objective: To discuss indications, contraindications, techniques, and clinical relevance of the expectant treatment (conven-tional and modified) and the ART technique in the treatment of carious lesions. Methods: A literature review of scientific articles (published in PubMED/Medline and Scielo data-bases, between 2009 and 2020) and books considered relevant to this study. Results: In the expectant treatment, carious dentin is removed in two stages: a superficial removal of the carious lesion at the first visit, and a final removal after different time intervals. However, some authors argue that it is not necessary to perform the second step. Just lower the glass-ionomer cements and then perform the definitive restoration, a technique known as modified expectant treatment, is necessary. Regarding the ART technique, the management of the lesion is conducted with the least possible intervention, using only manual instruments. It is reliable and well accepted especially in the Pediatric Dentistry clinic. Both culminate in favorable therapeutic results. Conclusion: Initially, the dentist must always carry out a thorough diagnosis of caries lesions aiming at the maximum prevention of the dental structure. You should opt for conventional restorative treatment only when other methods are no longer sufficient to stop the progression of the lesion.

Introducción: El síndrome de Marcus-Gunn se manifiesta con retracción o elevación del párpado ptótico ante la estimulación del músculo pterigoideo del mismo lado y el término "guiño mandibular" es inapropiado ya que el párpado no siempre desciende. Puede producirse en la apertura bucal, masticación, avance mandibular, al sonreír, silbar, bruxar, sacar la lengua, deglutir, chupar, cantar, en la contracción esternocleidomastoidea, maniobra de Valsalva, respirar o inhalar. Es congénito y de modalidad rara y autosómica dominante. Objetivo: Contribuir al conocimiento de esta rara enfermedad y a su relación con el ámbito odontológico. Presentación del caso: Se trata de un niño de 3 años diagnosticado de síndrome de Marcus-Gunn en el periodo neonatal, por la observación de la madre de una apertura palpebral izquierda durante la succión nutritiva. No presenta otras enfermedades ni antecedentes de interés. En la exploración extraoral se advierte una ptosis palpebral derecha y apertura palpebral izquierda en los movimientos mandibulares y deglución. La intensidad del reflejo se incrementa en estados de ansiedad derivados del tratamiento dental. Conclusiones: el síndrome de Marcus-Gunn es una entidad rara en pediatría, en la cual sus hallazgos clínicos determinan el diagnóstico. Teniendo en cuenta que en ocasiones presentan alteraciones oculares, nada nos hace sospechar la presencia de una enfermedad oral específica(AU)


Introduction: Marcus Gunn syndrome manifests with retraction or elevation of the eyelid ptotico while stimulation of the pterygoid muscle on the same side and the term jaw-winking is inappropriate because the eyelid does not always goes down. It can occur during mouth opening, mastication, mandibular advancement, while smiling, whistling, bruxing, sticking out the tongue, swallowing, sucking, singing, during the sternocleidomastoid contraction, the Valsalva maneuver, breathing or inhaling. This syndrome is congenital and rare, and of autosomal dominant modality. Objective: To contribute to the knowledge of this rare disease and its relationship with the odontologic field. Presentation of the case: 3 years old boy diagnosed with Marcus Gunn syndrome in the neonatal period by the observation of the mother of a left palpebral opening during the nutritive sucking. He does not present other diseases or a background of interest. In the extraoral exploration, it is noticed a right palpebral ptosis and a left palpebral opening in the jaw movements and in swallowing. The intensity of the reflex increases in anxiety states arising from the dental treatment. Conclusions: Marcus Gunn syndrome is a rare entity in pediatrics, in which its clinical findings determine the diagnosis. Taking into account that sometimes it presents ocular alterations, nothing makes us suspect the presence of a specific oral disease(AU)

ABSTRACT

Objectives: Reviewing information available about platelet-rich plasma (PRP) applied to dental treatments, introducing the general concept of PRP, as well as analyzing actual data about, and challenges faced by, the dental field. Data & sources: The current study analyzed the most informative publications about PRP application available in this field and gathered the maximum information about it as possible. Conclusions: PRP use, either alone or in association with other biomaterials, can significantly favor different fields such as tissue engineering, since it is an innovative technique that attracts the interest of clinicians and basic scientists. However, it is necessary conducting better designed and controlled experiments to enable successful tissue healing based on PRP use. Clinical significance: The current review can be used by clinicians as source of information about the actual rules and protocols adopted in the herein addressed field, besides providing specific examples of such applications. Objetivos: Revisar as informações disponíveis sobre o plasma-rico em plaquetas (PRP) aplicado a tratamentos odontológicos, introduzir o conceito geral de PRP e analisar dados reais sobre os desafios enfrentados pelo campo odontológico. Dados e fontes: O presente estudo analisou as publicações mais informativas sobre a aplicação do PRP disponíveis neste campo e reuniu o máximo de informações possível. Conclusões: O uso do PRP, isoladamente ou em associação com outros biomateriais, pode favorecer significativamente diferentes campos, como a engenharia de tecidos, uma vez que é uma técnica inovadora que atrai o interesse de clínicos e cientistas básicos. No entanto, é necessário realizar experimentos mais bem projetados e controlados para permitir a cura bem-sucedida dos tecidos com base no uso do PRP. Significado clínico: A revisão atual pode ser usada pelos médicos como fonte de informações sobre as regras e protocolos atuais adotados no campo aqui tratado, além de fornecer exemplos específicos de tais aplicações.(AU)

@#The goal of this retrospective study was to determine the types of dental treatments provided to paediatric patients under general anaesthesia (GA) at the Advanced Medical and Dental Institute, Universiti Sains Malaysia, Pulau Pinang, Malaysia. Records of the 113 patients under 18 years old who had undergone dental treatment under GA from November 2015 to February 2019 were retrieved and reviewed. The median age for subjects was six years old and the data were skewed to the left. Subjects were diagnosed with early childhood caries (n = 94) where total oral rehabilitation was performed for most subjects (n = 82). Higher number of deciduous teeth were extracted (n = 488) with average number per person of 5.5 as compared to permanent teeth (n = 43) with 2.9 average number per person. The leading cause of surgical cases was removal of supernumerary and impacted teeth. Tooth colour restoration and stainless-steel crowns were commonly used in restorative procedures. Although most of the patients were in the American Society of Anaesthesiology categories I and II, treatments were performed under GA because complete oral rehabilitation was required. In conclusion, dental treatment under GA is essential for providing complete oral rehabilitation in young patients, especially those with early childhood caries or medical and behavioural problems.

ABSTRACT

Background and Objective: There have been numerous studies of oral health status of school children and young population; however, similar studies in elderly population in India are lacking. With advances in medical science and consequent increase in life expectancy, elderly population is on the rise and is a subject of growing concern for public health policy. Hence, an attempt was made to study factors influencing decayed, missing, and filled teeth (DMFT) index, oral health awareness, and dental treatment-seeking behavior of elderly population. Methods: A cross-sectional community-based survey was conducted between September 2014 and December 2014 in villages in rural Maharashtra. Sociodemographic and health-related information were collected from 352 participants 60 years of age and above in 10 villages. Results: Prevalence of dental caries was 76.4% in a study population with median DMFT score of 12 with interquartile range of 7–22. The majority of the participants cleaned their teeth with fingers using charcoal and mishri. Only 17.2% participants used toothbrush. About 39% participants had experienced dental pain, of which majority did not visit dentist. The median DMFT index who used toothbrush and toothpaste was significantly less when compared with participants who did not use tooth brush and tooth paste. The majority of the participants had one or more missing teeth, but only 2.2% were using dentures. Conclusion: There is an urgent need for comprehensive oral health educational programs, and accessible and affordable oral health services to be provided to rural community.

Resumo O objetivo deste estudo foi avaliar associação entre dor pré-operatória, ansiedade e impacto da condição bucal na qualidade de vida dos pacientes atendidos no serviço de Urgência Odontológica da Universidade Federal dos Vales do Jequitinhonha e Mucuri. Estudo transversal retrospectivo foi conduzido avaliando dados clínicos de 240 prontuários de pacientes com idades entre 18 e 65 anos. A dor foi mensurada pela Heft-Parker Visual Analogue Scale; ansiedade, pela Corah's Detal Anxiety Scale e qualidade de vida, pelo Oral Health Impact Profile (OHIP-14). Associações foram analisadas pelo Teste qui-quadrado de Pearson (p < 0,05). Foi observada associação estatisticamente significante entre dor e pior qualidade de vida, com impacto nos domínios desconforto psicológico (p < 0,001), incapacidade física (p < 0,001), psicológica (p < 0,001) e social (p < 0,001). Ansiedade influenciou na pior qualidade de vida, com impacto nos domínios desconforto psicológico (p = 0,009), incapacidade fisica (p = 0,016), psicológica (p = 0,011) e desvantagem social (p = 0,028). Esse estudo é relevante para o planejamento de ações voltadas para a saúde bucal dos pacientes atendidos nos serviços de urgência odontológica, priorizando aqueles com maiores impactos psicossociais decorrentes dos problemas bucais.


Abstract The scope of this study was to assess the association between preoperative pain, anxiety and the impact of the oral condition on the quality of life of patients treated at the Dental Emergency Service at the Federal University of the Jequitinhonha and Mucuri Valleys. A retrospective cross-sectional study was conducted evaluating data from 240 patient charts aged from 18 to 65 years. Pain was measured by the Heft-Parker Visual Analog Scale; dental anxiety by Corah's Dental Anxiety Scale; and oral health-related quality of life by the Oral Health Impact Profile (OHIP-14). Associations were analyzed using Pearson's Chi-square test (p < 0.05). A statistically significant association was observed between pain and poorer quality of life, with impacts on psychological discomfort (p < 0.001), physical incapacity (p < 0.001) and the psychological (p < 0.001) and social (p < 0.001) domains. Anxiety influenced poorer quality of life, with impacts on the psychological discomfort (p = 0.009), physical disability (p = 0.016), psychological disability (p = 0.011) and social disadvantage (p = 0.028) domains. This study is relevant for the planning of programs and actions aimed at the oral health of patients treated in dental emergency services, prioritizing those with the greatest psychosocial impact arising from oral problems.

Abstract Background: Livedo reticularis is a benign dermatological condition characterized by ischemic areas permeated by erythematous-cyanotic areas in a lacy pattern, and may be transient or permanent and is frequently associated with body exposure to cold. Cutaneous arterial vasospasm promotes ischemia, and venous dilation of the congested areas occurs by tissue hypoxia or autonomic dysfunction. Patients with Down's syndrome, due to their physiological peculiarities, constitute a representative part of those who require dental care under general anesthesia, and livedo reticularis has a reported incidence of 8-12% in Down's syndrome patients. Objectives: To describe the physiological livedo reticularis in a Down's syndrome patient, with the onset during the anesthetic-surgical procedure. Case report: 5-year-old female patient with Down's syndrome, admitted for dental treatment under balanced general anesthesia with sevoflurane, fentanyl, and atracurium. Transoperative hypothermia occurred with axillary temperature reaching 34.5 °C after 30 min after the beginning of anesthesia. At the end of the procedure, red-purplish skin lesions interspersed with areas of pallor were observed exclusively on the ventromedial aspect of the right forearm, with no systemic signs suggestive of allergic reactions. The established diagnosis was physiological livedo reticularis. There was a total fading of the lesions within 5 days. Conclusion: This report evidences the need for thermal control of patients undergoing anesthesia, as well as the manifestation of livedo reticularis as a consequence of transoperative hypothermia.


Resumo Justificativa: O livedo reticular representa quadro dermatológico benigno, caracterizado por áreas isquêmicas permeadas por áreas eritematocianóticas em padrão rendilhado, pode ser transitório ou permanente e frequentemente é associado à exposição corporal ao frio. O vasoespasmo arterial cutâneo promove a isquemia e a dilatação venosa das áreas congestas e ocorre por hipóxia tecidual ou por disfunção autonômica. Os portadores da síndrome de Down, devido às suas peculiaridades fisiológicas, constituem uma representativa parcela daqueles que necessitam de atendimento odontológico sob anestesia geral e o livedo reticular tem incidência relatada de 8% a 12% em pacientes com síndrome de Down. Objetivos: Descrever quadro de livedo reticular fisiológico em paciente portador de síndrome de Down, com aparecimento durante o ato anestésico-cirúrgico. Relato do caso: Paciente de cinco anos, sexo feminino, síndrome de Down, admitida para tratamento odontológico sob anestesia geral balanceada, com emprego de sevoflurano, fentanil e atracúrio. Houve ocorrência de hipotermia transoperatória, a temperatura axilar atingiu 34,5 ºC após 30 minutos do início da anestesia. Ao término do procedimento, notaram-se lesões cutâneas vermelho-arroxeadas, intercaladas com áreas de palidez, exclusivamente na face ventromedial do antebraço direito, sem sinais sistêmicos sugestivos de reações alérgicas. O diagnóstico firmado foi de livedo reticular fisiológico. Houve esmaecimento total das lesões em cinco dias. Conclusão: O relato apresentado evidencia a necessidade de controle térmico de pacientes submetidos a anestesias, bem como registra manifestação de livedo reticular em consequência de hipotermia transoperatória.

ABSTRACT

Gingival probing performed in periodontal normal condition brings important results for the definition of health and diseases of the periodontal tissues. The aim was to evaluate the depth probing of the gingival sulcus in permanent dentition of young adults. It was carried out a transversal study with 120 volunteers aged 18 to 25 years and with healthy gum. It was used a manual periodontal probe Williams model. The evaluation was performed on all four sides, with a total of four measurements per tooth. The collected data were first tabulated and submitted to Kruskal-Wallis test with Dunn's post test.The mean and standard deviation of the upper teeth were: incisor (1.1343 ± 0.33665); canine (1.1819 ± 0.369); premolar ratio (1.3677 ± 0.3752); molar(1.8030 ± 0.4116). The mean of the lower teeth was: incisor (1.1260 ± 0.3272); canine (1.2106 ± 0.3390); premolar ratio (1.4580 ± 0.3778); molar (1.9068 ± 0.4497).It concludes that the average depth of the gingival sulcus, between the group of teeth was higher in the lower arch relative to the upper; except for faces: incisive distal, buccal molar and the lingual incisor and canine; in which the result related to the average depth is less than the upper arch. A sondagem gengival realizada em condições periodontais normais traz importantes resultados para a definição de saúde e doenças dos tecidos periodontais. O objetivo deste estudo foi avaliar a profundidade clínica do sulco gengival em uma dentição jovem. Um estudo transversal foi realizado com 120 voluntários entre 18 e 25 anos com gengiva saudável. Um modelo de Williams de sonda periodontal manual foi utilizado. A avaliação foi realizada nos quatro lados, com um total de quatro medidas por dente. Os dados coletados foram tabulados e submetidos ao teste de KruskalWallis com pós-teste de Dunn. A média e o desvio padrão dos dentes superiores foram: incisivo (1,1343 ± 0,33665); canino (1,1819 ± 0,369); pré molar (1,3677 ± 0,3752); molar (1,8030 ± 0,4116). A média dos dentes inferiores foi: incisivo (1,1260 ± 0,3272); canino (1,2106 ± 0,3390); pré-molar (1,4580 ± 0,3778); molar (1,9068 ± 0,4497). Concluise que a profundidade média do sulco gengival, entre o grupo de dentes, foi maior no arco inferior em relação ao superior; exceto por faces: incisiva distal, molar vestibular e incisivo lingual e canino; em que o resultado relacionado à profundidade média é menor que o arco superior (AU)

BACKGROUND: We aimed to assess the dose needed to achieve the propofol effect-site concentration using target-controlled infusion in intellectually disabled patients and to detail the most effective method for achieving a safe level of consciousness without hemodynamic changes as well as detail any resulting adverse effects. METHODS: We performed a retrospective review of sedation service records of 138 intellectually disabled patients (51, mental retardation; 36, autism; 30, brain lesion, 12 genetic diseases, 9 dementia) aged over 15 years and weighing over 30 kg. These patients had received propofol via target-controlled infusion in the special care dental clinic of Seoul National University Dental Hospital from May 2008 to September 2018 for restorative treatment (112), minor surgery (13), prosthodontics (7), periodontics treatment (5), and implant (1). RESULTS: For all groups, the duration of dental treatments was 43 ± 18 minutes, total sedation time was 73 ± 23 minutes, and total BIS values was 57 ± 12. The propofol maintenance dosage values for each group were: mental retardation, 3 ± 0.5 (2–4) µg/ml; autism, 3.1 ± 0.7 (2–5) µg/ml; brain lesion, 2.8 ± 0.7 (1.5–5) µg/ml; genetic disease, 2.9 ± 0.9 (1–4) µg/ml; and dementia 2.3 ± 0.7 (1–3.4) µg/ml. CONCLUSIONS: The dementia group needed a lower dosage to reach a safe, effective propofol effect-site concentration than the other groups. Since there were no complications, deep sedation is a great alternative to general anesthesia for dental treatment of intellectually disabled patients.

ABSTRACT Objective: To determine the profile of children from 6 to 12 years of age, in relation to anxiety towards dental treatment and evaluate the main factors that trigger anxiety in this population. Methods: The sample was composed of 200 children with an average age of 8.5 years who attended the Clinic of the School of Dentistry of the Federal University of Campina Grande and the Basic Healthcare Units of the municipality of Patos, Paraíba, Brazil between June 2015 and May 2016. Anxiety was assessed using the Dental Anxiety Scale and the Venham Picture Test. The Chi-Square test and Fisher's exact test were used to explore the level of significance of the associations between the variables Results: Anxiety was observed in the majority of children (Dental Anxiety Scale - 89% and Venham Picture Test - 70.5%) and the predominant levels were low to moderate. The child´s age group was significantly associated with anxiety (p=0.014) by the Venham Picture Test, while gender did not present this correlation. Anxiety was influenced by all the factors studied: making the appointment, waiting room, rotary instrument and periodontal scaling. Conclusion: The majority of children presented anxiety and the trigger factors: making the appointment, waiting room, rotary instrument and periodontal scaling contributed considerably to its development.


RESUMO Objetivo: Determinar o perfil das crianças de 6 a 12 anos de idade, com relação à ansiedade frente ao tratamento odontológico e avaliar os principais fatores desencadeadores de ansiedade nesta população. Métodos: A amostra foi composta por 200 crianças com idade média de 8,5 anos que frequentaram a Clínica-Escola de Odontologia da Universidade Federal de Campina Grande e as Unidades Básicas de Saúde da Família do município de Patos-PB entre junho de 2015 e maio de 2016. A ansiedade foi avaliada utilizando a Dental Anxiety Scale e o Venham Picture Test. O teste Qui-Quadrado e Exato de Fisher foram usados para explorar o nível de significância das associações entre as variáveis. Resultados: A ansiedade foi observada na maioria das crianças (Dental Anxiety Scale - 89% e Venham Picture Test - 70,5%) e os níveis predominantes foram baixo e moderado. A faixa etária da criança foi associada significativamente à ansiedade (p=0,014) através do teste Venham Picture Test, ao passo que o gênero não apresentou essa correlação. A ansiedade sofreu influência de todos os fatores estudados: marcação da consulta, sala de espera, caneta odontológica e raspagem periodontal. Conclusão: A maioria das crianças apresentou ansiedade e os fatores desencadeadores marcação da consulta, sala de espera, caneta odontológica e raspagem periodontal atuaram consideravelmente para o seu desenvolvimento.

Abstract Background and objectives 15q tetrasomy is a chromosomal abnormality that is a part of the heterogeneous group of extra structurally abnormal chromosomes. This syndrome is characterized by epilepsy, central hypotonia, developmental delay and intellectual disability, and autistic behavior. This is the first report of the anesthetic management of a patient with this syndrome. Case report We administered general anesthesia for dental treatment in a patient with 15q tetrasomy. Conclusions Appropriate planning for the prevention of complications such as seizures and hypotonia, and for delayed emergence from anesthesia, is required. Specifically, choosing short-acting drugs that do not induce seizures, together with suitable monitoring, resulted in successful anesthetic management of the patient with 15q tetrasomy.


Resumo Justificativa e objetivos Tetrassomia 15q é uma anomalia cromossômica que faz parte do grupo heterogêneo de cromossomos extras, estruturalmente anormais. Essa síndrome é caracterizada por epilepsia, hipotonia central, atraso no desenvolvimento e deficiência intelectual e comportamento autista. Este é o primeiro relato do manejo anestésico de um paciente com essa síndrome. Relato de caso Administramos anestesia geral para tratamento odontológico em um paciente com tetrassomia 15q. Conclusões Um planejamento adequado para prevenir complicações como convulsões e hipotonia e para emergência tardia da anestesia é necessário. O manejo anestésico bem-sucedido do paciente com tetrassomia 15q foi o resultado específico da escolha de fármacos de curta duração que não induzem convulsões e monitoração adequada.
